What to Expect

When you visit our office, you will be welcomed, cleared through our COVID screening, and our front desk can answer any questions you may have about insurance and patient registration.

Your doctor will lead a team that will work together to evaluate your vision, eye health and provide the best treatment option(s) for your condition(s). Our opticians will review your vision benefits and payment options, fit you for glasses and assist with any contact lens consult services. If at any time you have questions during the process, please ask.

We recommend you plan to spend about 60-90 minutes at your appointment depending on the complexity of your needs.

What to do before you appointment

COVID precautions

  • Due to COVID, if you are coming for medical issues related to the health of your eyes like diabetes, cataracts, glaucoma, red eyes, irritated eyes, floaters or flashes our team will attempt to call you in advance for background information and to reduce the time you are in the office.

Insurance Questions

  • If you have specific insurance questions prior to your visit, please contact our insurance department at 901-872-2020 extension 234 at least 2 days prior to your appointment.

 

Appointment Confirmation Required

  • Confirm with our team that your mobile number and email address on file are correct. You will receive reminders via email and text. Unfortunately, 90% of patients who do not confirm their appointment do not show up for their appointment. Therefore, we require you to confirm your appointment electronically or via phone at least 24 hours in advance. Otherwise, the appointment will automatically be cancelled to allow for another patient to be seen.
  • After confirming your appointment, avoid the no show fee by cancelling at least 24 hours in advance. 

 

Patient Record Portal Registration

  • West Tennessee Eye is pleased to offer you convenient, secure access to  your electronic health records through our online portal called RevolutionPHR.
  • This online portal gives you instant access to your secure communications with your doctor, billing account information, appointment history, upcoming appointments, and status of contact lens and glasses orders. 
  • RevolutionPHR utilizes the highest standards of online security while allowing you to view portions of your eye care record through any standard internet browser on a PC, Mac, or IPad.
  • We encourage you to create your account now.
    • www.revolutionphr.com
    •  Insert the username and temporary password listed below:
      • Username is your first name initial and last name 4 digits of birth year (no spaces, all lower case)
      • For example: John Smith born in at 1960 would be: jsmith1960
    • TemporaryPassword is 
      • eyeball2020 (all lowercase)
  • This process will lead you to a reset password screen where you will be prompted to re-enter the temporary password of “eyeball2020” and then input your new password that must be at least six characters long, including at least one number and one capital letter.
  • If you should ever need to reset your password, the system will send you a link to the email address you originally put on file with our office.

Patient Registration Form

In order to ensure that your first visit goes smoothly, please complete the forms below prior to your visit. This information is needed to comply with state and federal laws, to file your insurance and to help your doctor understand more about you, your vision and your overall medical health. We recommend you fill it out online versus downloading it and bringing the paper copy. Having this information in advance will decrease your wait time.

Complete this form if you are coming for only glasses and contact lenses. This form allows us to file your vision insurance like Vision Service Plan, EyeMed, Davis Vision, Spectra and TennCare for school aged children.

Payment Options

West Tennessee Eye believes in providing the best value for you and your family. We have products and pricing that aims to accommodate all budgets and our office participates in most insurance programs. For patients who have little or no vision insurance coverage, flexible payment programs may be arranged. Our office accepts cash, check, and major debit and credit cards including MasterCard and Visa, as well as Care Credit.

Don’t forget to use your:

  • Cafeteria Plans for most of your eye care needs

All of the above plans can be used for all your eye care and eyewear needs, including eyeglasses, contact lenses, routine office visits and medical office visits.

Care Credit

Insurance

Since there are so many plans and coverage varies widely, please call us to find out if your plan covers care in our office. Our staff will verify and explain your benefits before your visit at no charge.
